Bridging Gaps in Equity and Access: A Localized ReSurge Curriculum in Africa

What is the difference between the curriculum accessible to residents at Stanford Medicine’s Division of Plastic and Reconstructive Surgery and residents living in Eastern, Central, and Southern Africa? Less than you would think, thanks to an exciting new ReSurge initiative to bring equity, access, and localization to reconstructive surgical training across Sub-Saharan Africa.

This project has the potential to have a massive and long-term ripple effect throughout the region. What’s on the table is a complete ReSurge supported and localized global reconstructive surgery curriculum for our African partners and all reconstructive surgery trainees across 14 different African countries.

Dr. Lawrence Cai, ReSurge’s current Laub Fellow and reconstructive surgery resident at Stanford Medical School, is spearheading this project along with ReSurge Consulting Medical Officer and Chief of Stanford’s Plastic and Reconstructive Surgery Division, Dr. James Chang.

We caught up with Dr. Lawrence Cai to learn more about the roots, evolution, and upcoming launch of this project. Take it away Dr. Cai!

Sub-Saharan Africa has the most acute shortage of reconstructive surgical care in the world with only one reconstructive surgeon for every 10 million people. To confront this deficit, the ReSurge International Program in Africa (RIPA) provides training, curriculum development, scholarships, regional study exchanges, exam and membership fees, and eLearning centers for places of study. We conduct this work as collaborators and proud partners with the College of Surgeons of East, Central, and Southern Africa (COSECSA).

In early 2021, team members from ReSurge, COSECSA, and the Royal College of Surgeons of Ireland met to discuss the unmet needs for reconstructive surgery trainees in Africa and how our groups could address those needs. One of the identified needs was a core reconstructive surgery curriculum that was specific for low- and middle-income countries and included local discussion cases from Africa. From that collaborative meeting, we set an ambitious goal:

To set up COSECSA’s first-ever comprehensive reconstructive surgery curriculum and lecture series.

Our work on the first comprehensive online reconstructive surgery curriculum is now nearing completion, and we aim to have all of the materials ready for the next generation of COSECSA trainees by the Fall of 2022! At which point every reconstructive surgical trainee across the 14 COSECSA countries will be using a ReSurge supported curriculum. Stay tuned for more updates as the curriculum launches.

The vision for this curriculum was that it could serve as the foundational didactic material for a reconstructive surgery trainee, upon which he or she would then continue to build clinical expertise through time in the wards and the operating theater.

To identify the critical topics that make up the foundation of reconstructive surgery, we turned to several seminal reconstructive surgery textbooks across a range of levels, as well as the contents of Stanford’s annual board exam review course and the American Council of Academic Plastic Surgeons (the group that writes the annual in-service exams for the residents). From these sources, we looked for topics that consistently appeared at all levels. Ultimately, we identified a total of 26 topics that cover the entire head-to-toe spectrum of reconstructive surgery, ranging from cleft lip repairs to peripheral nerve injuries.

With these topics in hand, we then partnered with reconstructive surgeons from top US-based universities and private practices to create online lecture modules. Each module consists of a content-expert reconstructive surgeon narrating a 60-90 minute lecture that serves as a primer on the subject and includes material on the relevant surgical anatomy, common injury patterns, and key reconstructive techniques, as well as test questions to assess comprehension.

In November, we had the opportunity to travel to Zimbabwe, our first time in the field since the start of the pandemic. Following the COSECSA reconstructive surgery oral exams, ReSurge hosted the COSECSA Program Directors’ Meeting. At that meeting, we had a chance to share the framework of the curriculum with the program directors who would ultimately be using these modules with their trainees.  We were thrilled at the positive feedback on the early progress of the work and the enthusiastic engagement from all of the program directors to help add to the content. We also gained incredibly valuable feedback on additional areas of reconstructive surgery that were not included in the initial list of modules—such as topical diseases pathologies, management of surgical infections, and research skills—that will be included in a 2.0 version of this curriculum.

One of the goals that was identified at the inception of this project was to create content that is specific to the Africa training context. It was critical to include examples of local reconstructive surgery patients. To that end, the program directors were also invited to draw upon their own surgical experience and contribute sample cases from their clinical practice. As the lecturers wrap up their modules, our work is now shifting towards collecting these local cases and preparing the modules to be released online.

Sujana is a six-year-old who lives in the outskirts of the Katmandu Valley in Nepal with her parents and two older brothers. They live in a small hut in the hills among the fields that they farm. Sujana is a bright young girl with an infectious and somewhat mischievous smile. She loves to draw and is always sketching beautiful scenes of mountains, flowers, houses, and people.

Life for Sujana’s family had always been hard, but they always had hope and resilience. Originally from the city, they moved to the hills to make a better life for their young family. Sujana’s mother Anita works as a sharecropper and her father works as a day laborer to make ends meet.

After just six months into their new life, tragedy struck.

It started off just like any other evening. Anita left the house with vegetables to sell at the local market, leaving her elderly mother-in-law at home with the children. They left a candle in the house as a night light.

That night the house caught aflame with the children and the grandmother inside.

Sujana’s two older brothers were lucky and escaped, and their grandmother also made it out of home crawling on her hands and knees. But Sujana was stuck.

“I was 500 meters from the house when I saw my home catch on fire,” recalled Anita. “My heart sunk. I thought I had lost my children. I thought my life had ended. My whole world was in shock and I ran as fast as I could screaming for help. A neighbor boy ran into the house and saved my youngest, Sujana. He was wearing a winter coat and it caught fire as he ran out of the door with my daughter.”

Sujana’s life was saved by her neighbor. The boy was only slightly burned, but Sujana sustained burns on her arms, face, foot, and leg.

For two months Sujana and her mother bounced between hospitals, trying to get Sujana the care she needed to heal her burns. Then tragedy struck the family again when Sujana’s grandmother fell ill and eventually passed away. The family had to return back home and pay for the funeral costs.

“We had nothing left, I was totally out of money.”

That’s when Anita found out about ReSurge International, and the free care available through Dr. Shankar Rai, ReSurge’s Nepal Program Director, at The Nepal Cleft & Burn Center.

By the time Anita found ReSurge, it had been two years since the accident. Sujana’s burns had contracted, leaving her hand and foot severely deformed from the scar tissue. When a burn is left to heal on its own, tightening scar tissue can disfigure and ruin the function of an appendage: a hand contracts into an immovable fist; or a foot fuses to the shin, making it impossible to walk.

After finding ReSurge, Sujana was able to get her first set of free reconstructive surgeries to heal her contractures, plus weekly check-ups and physical therapy so that she can write, draw, and go to school again.

Anita wants her daughter to become a doctor one day, just like Dr. Rai.

“I moved here with the hope to make a better life for my children. I thought those dreams had been lost in the flames. But I saw how [ReSurge] took care of the burn patients and if my daughter can one day be a doctor, that would be my dream for her.”

Tragedy struck Anita and her family, but thanks to Dr. Rai and the compassionate ReSurge supporters who fund our work, Sujana rose like a Pheonix towards a life of hope and promise.

11 million people are burned every year. In the developing world, where over 96% of fatal fire-related burns occur, Sujana’s story is all too common. Why? Poverty. Nearly half of the world still uses open flames for cooking, heating, or lighting. Women and children are at particular risk. Those who survive their burns are often permanently disabled, costing more than $4 billion per year in lost productivity.

ReSurge International’s mission is to help families like Anita’s around the world by offering free reconstructive surgical care to patients in need, and by training the next generation of local surgeons like Dr. Rai. ReSurge doesn’t just treat burns, we also treat the full scope of reconstructive care for the child who fell into a cooking fire, the mother who was injured in a motorcycle accident, or the son who was born with a congenital anomaly like a cleft lip or a malignant condition like cancer.

At the end of the day, transforming the lives of our patients is what we are all about. Sujana, is a perfect example of a child full of hope and dreams. When we asked Sujana to tell us about herself, this is what she said.

“My favorite thing to eat is eggs and my best friend is Jiban,” said little Sujana as she drew pictures.

“I am brave and not scared of the hospital. Well, I am a little scared when they do surgery.”

Meet Jackson. This exuberant boy took a ten-hour bus ride with his mother to our clinic in Tanzania. Jackson had a hairy nevus, a congenital condition that caused raised black bumps to grow on his skin. Whenever he fell or was just nudged, the nevus would break open and start weeping, creating more bumps. Even worse? There was an extremely high likelihood it would lead to cancer.

Accidents, burns, and congenital problems like Jackson’s can keep people from leading a productive life, creating an enormous burden on their families and wider communities. However, reconstructive plastic surgery can transform lives in just a few hours. In fact, the World Bank has found that surgical procedures are among the most cost-effective health interventions in the developing world.

Our surgeons removed the entire nevus from Jackson’s face and replaced it with a skin graft. Jackson high-fived the medical team, and his mother was overwhelmed with relief: “I have been worried about Jackson since he was born. ReSurge has given me hope that he will finally be treated forever.”

A Bright Future for Philip

Philp is a bright-eyed toddler who loves playtime with his siblings. Philp was born in Uganda, a country with a population of over 45 million but only 0.73 general surgeons per 100,000 people. The need for reconstructive surgeons is exceptionally high in low-income countries, particularly in Sub-Saharan Africa. With so few surgeons, it can be rare to find a reconstructive surgeon with the specific skills necessary to treat difficult cases. The ideal goal should be 20 specialist surgeons, anesthesiologists, and obstetricians per 100,000 people.

Reconstructive surgery treats congenital anomalies like clefts, traumatic injuries including burns and road traffic accidents, and malignant conditions like cancers. These conditions represent approximately 30% of the global burden of disease. That’s three times more than malaria, tuberculosis, and HIV/AIDS combined.

The lack of safe, timely, and affordable surgical care can be disastrous for communities, families, and children. Philp and his family know this truth all too well.

Philip’s mother is a food vendor. One unlucky day, Philp’s 10-year-old sister was helping her mother move a large, boiling pot of beans when the pot slipped and fell all over baby Philp. Over 40% of his body was scalded.

Not knowing what to do, the family attempted to treat him at home for two months. His condition worsened. When he was finally brought to the hospital, Philp was septic, anemic, and with an electrolyte imbalance. After three weeks of intensive care, Philp was stable enough to receive a skin graft.

Without medical intervention, Philp may have become one of the millions of people who die each year from surgically preventable causes.

Philip’s Surgeon, Dr. Seif Nuru

Dr. Seif Nuru, the ReSurge surgeon who operated on Philp, is a general surgeon from Tanzania currently receiving reconstructive surgical training funded by ReSurge in Uganda. Dr. Nuru is part of ReSurge’s initiative to train the next generation of local surgeons and surgical teams in low-income countries to create a ripple effect of impact and long-term solutions for the lack of surgical care in low-income countries. In many countries where we work, we are training the first and only reconstructive surgeons in the entire region.

ReSurge’s mission is to transform lives by building surgical capacity where it’s needed most, so more children like Philp don’t have to suffer. Philp has been through a lot at a young age, but because he got the care he needed, we know that his future is going to be as bright and promising as the sparkle in his eyes!

Our First Trip in 2021: Part Two in Zimbabwe

Over the past year, ReSurge was not able to facilitate international travel or in-person training due to the pandemic. Despite the setbacks, we were able to create a robust virtual learning program and kick-off our ReSurge International Program in Africa (RIPA) in full. Even so, we were still eager to get back out into the field. And so, a small team of ReSurge staff and volunteers made their first voyage back in November to see some of our local partners.

Last week, Chief Program Officer Natalie Meyers, shared a reflection on the first part of that adventure–a trip to Kenya to meet our newest Surgical Outreach Partner. Read on for the next adventure in that journey–a trip to Zimbabwe to meet the next generation of reconstructive surgeons and ReSurge scholars in Eastern, Southern, and Central Africa!

Zimbabwe & The Program Directors Meeting

After a successful week in Kenya, the second leg of our trip we headed to Zimbabwe to support the College of Surgeons of Eastern, Southern, and Central Africa (COSECSA) to conduct their plastic and reconstructive surgery oral exams for trainees from across the continent and to host the COSECSA Program Directors Meeting.

Thanks to generous RIPA donations, ReSurge was able to sponsor Dr. Dagmawi Desta, Dr. Ephrem Tekaligne, and Dr. Fikru Sima to cover their exam fees, travel costs, and, with the successful passing of their boards, 5 years of COSCESA membership. All three Ethiopian surgeons were incredibly grateful to be able to attend and take their exams, as this last hurdle to becoming a certified reconstructive surgeon had eluded them in the previous year due to lack of funding.

Two additional ReSurge one-month study exchange students also sat for their boards, one student from Ethiopia and another from Tanzania. Dr. Adeliade Mghase, the Tanzanian ReSurge scholar, will be the first female reconstructive surgeon in her country. Dr. Owen Muzinda, from Zimbabwe, one of Dr. Chang’s mentees, received the highest board score in the region. This will be the second year in a row that a ReSurge mentee wins this highest honor, with Dr. Tinga Nyoni receiving the award last year. Everyone was thrilled that all 10 trainees passed their exams, and thus, 10 new reconstructive surgeons in the ECSA region!

While in Zimbabwe for the exams and meeting, the team was also able to see ReSurge’s newest eLearning Center at Parirenyatwa Hospital. The center was generously donated in honor of Rick Compton, and we are looking forward to continuing Dr. Chang’s monthly case conferences so that we can keep our streak of churning out the highest performing reconstructive surgical trainees in the region!

Honoring ReSurge’s Newest eLearning Center at Parirenyatwa Hospital

After the conclusion of the exams, ReSurge sponsored and hosted the 11 COSECSA Reconstructive Surgery Program Directors from across the six countries with training programs. It was an incredible opportunity for these founding doctors in reconstructive surgery in their respective countries to convene in one place. Dr. Chang, ReSurge’s CMO and Chief of the Division of Plastic and Reconstructive Surgery at Stanford Medicine, rallied an impressive group of virtual attendees and prestigious American academic partners representing Northwestern, University of Washington, Johns Hopkins, and Stanford Medicine. The attendees discussed the current landscape of reconstructive training and residency programs in each of their countries and hospitals. The group further worked on the development of the COSECSA-wide reconstructive surgery online curriculum that will involve 26 modules, each with 60-90 minute recorded lectures, 10 test questions, and local cases for discussion. This work is being led by ReSurge’s current Laub Fellow, Dr. Lawrence Cai who was also able to join for the Zimbabwean portion of our trip. Stay tuned for an upcoming blog post about this work!

The COSECSA Reconstructive Surgery Program Directors Meeting

In collaboration with our US academic partners and the COSECSA Program Directors, the plan is to complete the curriculum and to have it on the COSECSA eLearning platform by the end of 2022. ReSurge’s curriculum will now be the foundation for all trainees across the entire region. At the conclusion of the meeting, all trainees, now newly minted reconstructive surgeons, joined their Program Directors in a celebratory dinner at the Rainbow Towers Hotel in Harare. ReSurge looks forward to hosting an in-person Train the Trainers Conference again in 2022.

The Last Unexpected Obstacle: Omicron

As we sat back satisfied with the success of our trip and just starting the long journey back to the US, at our very first layover in South Africa more COVID news broke out. There we sat on the tarmac for 90 minutes while flight attendants in frantic whispers, tried to figure out if our flight would still be allowed into the UK as the world had just announced the latest discovery of the Omicron variant and a new string of travel bans. Thankfully our South African layover was one of the very last flights allowed into the UK and eventually back to the US. All travelers isolated when they were home, tested regularly, and are safe and COVID-free. Given the new travel restrictions for Africa, the ReSurge volunteers and staff were especially grateful to get to visit our partners and colleagues in person, making the return to Zoom calls all that much easier to continue our successful partnership and work in Africa.
